School Forms

Except where noted, the following forms must be submitted on or before August 1, 2022.

Some are submitted via the Parent Portal. For assistance, contact Bree McNamara, our director of admissions. Others are in pdf format and, once completed, can be emailed or delivered to Sarah Smith-Centz, our administrator for community support.

COVID Compact
This Form will be available by August 31 and will be due September 7. Reminders will be sent Miquon Matters.
Parent Portal The Portal will prompt you to submit one form per family.
Physician’s Report of Medical Exam PDF link
Submit via email or hand deliver to Sarah Smith-Centz.
Required for all new students, students entering kindergarten, and students entering 6th grade. We can accept reports dated July 1, 2021 or later. New reports are due by December 1, 2022.

Note: proof of immunizations must be submitted on or before September 6, 2022. No exceptions.

Dental Report PDF link
Submit via email or hand deliver to Sarah Smith-Centz.
Required for all new students (except nursery), and all students entering kindergarten or 3rd grade. We can accept reports dated July 1, 2021 or later. New reports are due by December 1, 2022.
Release of Prior School Records (New Students Only) PDF link
Submit this form to your child’s last school to authorize release of health and academic records.
Required for all new students Grades 1 – 6.
Transportation & After Care Form Parent Portal Due upon enrollment, this form advises us if you need bus, van, or After Care services. If there have been changes to your original plans, please reach out to Bree McNamara. If services are needed, appropriate contracts must also be submitted. See below. Visit our Extended Day Programs page to learn more about that service.

Medication Permission Form  PDF link
Submit via email or hand deliver to Sarah Smith-Centz.
Required IF you want school staff to administer ANY medication during the school day or school-supervised activities (i.e., overnight trips). Your child’s doctor must sign. Includes asthma, prescription, OTC and allergy medications.
Asthma Action Plan PDF link
Submit via email or hand deliver to Sarah Smith-Centz.
If your child has asthma, you must submit a physician-approved action plan in addition to a Medication Permission Form if medication is a part of the plan.
Food Allergy Action Plan PDF link
Submit via email or hand deliver to Sarah Smith-Centz.
If your child has a food allergy, you must submit a physician-approved action plan as well as a Medication Permission Form if medication is a part of the plan.
After Care Contract Parent Portal Due July 15th. The Portal will prompt you to submit one form per child. Please contact Bree McNamara with questions.
Van Contract DocuSign If you requested van service, you should have received an email from Bree McNamara containing a DocuSign link to your contract. Please contact Bree with questions or if you did not receive that link.
