2019-20 School Year Forms
The following forms are required for all students enrolling for the 2019-20 school year. Please return these forms as soon as possible or by August 16.
Parent Portal Forms

- 2019-20 Student Information Form (one per enrolled child)
- 2019-20 Regular Dismissal Routine (one per child)

- 2019-20 Requirements for Driving Form (optional; one per driver)
- 2019-20 After Care Contract (optional; one per child)
- 2019-20 Van Contract (optional; one per child)
Physician’s Report of Medical Exam
Required for all new students, students entering Kindergarten, and students entering 6th grade
Required for all students entering Kindergarten, entering 3rd grade, and all new students (except Nursery).
Medication Permission Form
REQUIRED for all students if you would like the school staff to give your child medication during the school day or school-supervised activities at any point. This might include prescription medications that may need to be given on overnight field trips. This might also include over-the-counter medications such as Benadryl or Motrin that your child may need now or in the future on an as-needed basis for intermittent concerns. Intermittent concerns may include but are not limited to seasonal allergies and/or swelling and/or pain, such as when your child has had orthodontic work or a sports injury. This form requires a signature from your child’s pediatrician.
Asthma Action Plan and Food Allergy Action Plan
If your child has a serious medical condition that may require immediate treatment and/or daily management at school, you must submit a physician-approved action plan in order for your child to attend school. You must also submit a Medication Permission Form (above) if medication is a part of your doctor-approved action plan.
