Required Forms

The following forms are required for all students enrolling for the 2018-19 school year. Please return these forms as soon as possible.

Parent Portal Forms
Required for all students. Please complete the following Parent Portal forms by logging into the Parent Portal. If you need assistance logging in, contact Pam Feinschil by emailing pamf@miquon.org or calling 610.828.1231. 

  • 2018-19 Student Information Form (one per enrolled child)
  • 2018-2019 Family Information Form (one per family)
  • 2018-2019 Automated Emergency Announcement Form
  • 2018-2019 Family Emergency Contact Form
  • 2018-2019 Requirement for Driving Form

Physician’s Report of Medical Exam (updated)
Required for all new students, students entering Kindergarten, and students entering 6th grade

Medication Permission Form
REQUIRED for all students if you would like the school staff to give your child medication during the school day or school-supervised activities at any point. This might include prescription medications that may need to be given on overnight field trips. This might also include over-the-counter medications such as Benadryl or Motrin that your child may need now or in the future on an as-needed basis for intermittent concerns. Intermittent concerns may include but are not limited to seasonal allergies and/or swelling and/or pain, such as when your child has had orthodontic work or a sports injury. This form requires a signature from your child’s pediatrician.

Miscellaneous forms required from some students and due before the opening of school. Please check which forms apply to you (you may use either the forms provided here or the forms provided by your health care providers).

  • 2018-19 Van Contract

Dental Report
Required for all students entering Kindergarten, entering 3rd grade, and all new students (except Nursery).

Asthma Action Plan and Food Allergy Action Plan
If your child has a serious medical condition that may require immediate treatment and/or daily management at school, you must submit a physician-approved action plan in order for your child to attend school. You must also submit a Medication Permission Form (above) if medication is a part of your doctor-approved action plan.

Release of Records
For all new students in 1st grade and above.
